WHO PERFORMS ENMG

Accurate diagnosis starts with the doctor’s experience

  • ENMG involves more than measuring nerve impulses.
  • It is important to choose the right testing method, evaluate the results and relate them to the patient’s symptoms.
  • At SPRAVNO, the test is performed by Oksana Mytsak, a neurologist and functional diagnostics specialist experienced in electroneuromyography and the diagnosis of peripheral nervous system disorders.

WHAT TO EXPECT

What happens during ENMG?

Up to 2 hours

Depending on the type and scope of the test

  1. 01

    Before the test begins

The doctor reviews your referral, if you have one, asks about your symptoms and reviews previous test results. If needed, the doctor clarifies the scope and method of ENMG.

  2. 02

    Nerve conduction studies

The doctor places electrodes on your skin and uses brief electrical impulses to test nerve conduction. You may feel tingling and brief muscle contractions during the test.

  3. 03

    Needle EMG — when indicated

To assess electrical activity in the muscles, the doctor may use a thin, single-use needle electrode. The test may cause temporary discomfort and is performed only with your consent.

  4. The doctor’s conclusion

After the test, the doctor analyses the results and prepares a medical report. The findings help clarify the diagnosis and guide further treatment.

02 · I HAVE NO REFERRAL

Start with a neurologist consultation

Numbness, tingling or weakness alone do not mean that you need ENMG.

The doctor will determine whether ENMG is needed and the scope of the test.

A neurologist consultation is a separate service from 1550 UAH.

The cost of ENMG depends on the method and scope of the test and is charged separately.

  1. Prepare your questions

What is troubling you? Bring previous test results and a list of your medicines.

  2. Arrange a consultation

Check the doctor, cost and a convenient time with the administrator.

  3. Discuss whether the test is needed

The doctor determines the type and scope of ENMG. You do not need to choose a testing method yourself based on symptoms.

When might ENMG be appropriate?

Numbness, tingling or weakness in the arms or legs may indicate peripheral nerve dysfunction. Electroneuromyography helps identify which nerves or muscles are affected and the extent of the changes. These examples do not establish a diagnosis; the doctor determines the type and scope of the test.

IF YOU HAVE A SPECIFIC SITUATION

Answers for
specific situations

For military personnel and veterans
  • SPRAVNO offers special terms for military personnel. If you are serving or a veteran, ask the administrator which terms apply to your status specifically for ENMG, which documents to bring and whether discounts can be combined.
  • The doctor who performs ENMG co-authored a study of ENMG in the consequences of gunshot and mine-blast injuries to limb nerves (Ukrainian Neurosurgical Journal, 2024).
  • Signs of nerve injury appear on ENMG 2–4 weeks after injury; recovery is assessed with repeat tests, usually after 3–4 months.
  • If you have a referral from a military hospital or are preparing documents for a military medical commission, ask the administrator in advance about booking with that referral and the documents available. Check the commission’s report requirements directly with the commission.
Having repeat ENMG for follow-up
  • Repeat ENMG is performed to assess changes over time: after an injury, with new symptoms or if the first test was incomplete.
  • Bring previous ENMG reports: the medical conclusion describes changes compared with the previous test.
  • Comparison is easier when the same method is used for repeat testing: different examiners obtain noticeably different values.
  • If your previous ENMG was performed elsewhere, mention this when booking. Ask the administrator how to send the doctor a question about the previous report and the prescribed scope of follow-up testing.
When surgery is being considered
  • ENMG helps clarify the exact location and severity of a problem. MRI shows structure, while ENMG shows nerve and muscle function, so neither replaces the other.
  • Bring your MRI, neurologist or neurosurgeon reports and previous ENMG reports.
  • You can discuss the result with the centre’s neurosurgeon at a separate consultation, from 2650 UAH. Ask the administrator about the earliest appointment, whether a consultation is possible on the day of ENMG and when to expect a reply. Also ask about a paper or electronic copy for your doctor.

BEFORE YOUR VISIT

How to prepare
for ENMG

No complicated special preparation is needed.

What to bring
with you

  • Referral
  • Previous ENMG reports, neurologist reports and other test results
  • A list of the medicines you take
THIS HELPS DETERMINE THE SCOPE
  • Clean skin

On the day of the test, do not apply creams, oils or lotions to the areas being examined.

  • Loose clothing

To allow easy access to your arms and legs.

  • Warm limbs

In cold weather, wear gloves and warm socks: cooling slows conduction and affects the results.

  • Eat as usual

You do not need to fast.

  • Do not stop medicines on your own

If the test is to diagnose myasthenia, ask your doctor in advance how to take your medicines.

  • Tell the doctor

About a pacemaker, defibrillator or neurostimulator, medicines that reduce blood clotting, and any wounds or rash in the area being tested.

BEFORE BOOKING

Frequently asked
questions

Does it hurt? Can it be done without a needle?

Nerve conduction studies use brief impulses: uncomfortable, but usually not painful. Needle EMG is performed only when needed; needle insertion causes discomfort like an injection, and the muscles may then feel tender for a few days. It is not performed without your consent. If anything is uncomfortable, say so and the test will be stopped.

How can I check the price for my referral?

1840 UAH is the price for “Upper limb nerve conduction studies, including F-waves”. When booking, tell us what your referral says and check whether the price covers one or both limbs and what scope is included. The doctor determines the type and scope of the test; check the cost of any additional testing before it is performed.

Do I need a referral?

ENMG is always prescribed by a doctor. If you have a referral, bring it. Without a referral, start with a neurologist consultation. At SPRAVNO it costs from 1550 UAH and is charged separately.

Who will explain the result to me?

The doctor performing the test will explain it during the examination. You receive the medical report immediately. A treatment plan is drawn up at a separate consultation.

What does a normal result mean?

Studies in ENMG laboratories report normal results in 25–38% of patients. This is also an answer, but not always the final one: for example, standard ENMG does not detect small-fibre nerve damage, and results can be normal in radiculopathy. The doctor decides what to do next.

Can ENMG detect multiple sclerosis?

No. ENMG tests peripheral nerves and muscles; it does not assess the brain or spinal cord. Multiple sclerosis is diagnosed using MRI, cerebrospinal fluid analysis and other methods. If this concerns you, start with a neurologist consultation.

Do I need ENMG or MRI?

These are different tests. MRI shows structure: the spine, disc herniations and tissues. ENMG shows how nerves and muscles function. MRI often detects changes that cause no symptoms, and it does not assess polyneuropathy. The doctor decides what you need.

Can I have the test with a pacemaker or while taking blood-thinning medicines?

Tell the doctor in advance. Stimulation is usually safe with modern pacemakers; medicines that reduce blood clotting are not routinely stopped — the doctor makes that decision. Significant bleeding after needle EMG occurs in fewer than 1 in 10 000 tests.

When can I come in?

Check the ENMG examination room’s schedule and a convenient time with the administrator: (044) 344 20 20.

Which tests does the 20% discount apply to?

The 20% discount applies to all ENMG tests until 31 October 2026. The discount is included in the listed prices. Check eligibility and the appointment date to which the discount applies when booking.

When should I seek urgent medical help?

Sudden weakness or numbness in an arm or leg, facial asymmetry, or speech or vision problems may be signs of a stroke. Do not wait for a scheduled ENMG test. Call an ambulance immediately on 103 or 112.

Reviews of SPRAVNO clinic and its doctors

Reviews

I was referred to this center by my neurologist as a place where I could get help with my chronic migraine. I booked a consultation with Nataliia Mykhailovska. She has extensive experience as a psychotherapist and neurologist. During the consultation, we discussed in detail all the issues that were bothering me, after which she drew up an individual 1-month course of treatment. Every 2 weeks I send a report on how my body is reacting to the medications (there is a special pain diary where I record the current indicators of my condition every day), meaning the treatment is constantly adjusted according to my condition.
Result: my condition has improved significantly. My sleep has become better, my anxiety has decreased, and the headaches are no longer as exhausting as before.
The girls at the reception are always in touch, coordinate everything, and answer all questions – I feel their professional approach at every level.
From myself I want to say: if you have headaches more often than 1-2 times a month – this can and should be addressed. It is possible to ease the condition and shorten the duration of the attacks themselves. Don’t endure it – see a specialist.
Thank you to the center and especially to Nataliia Oleksandrivna 🙏

I came in with several problems at once: a pinched nerve after surgery and wrist inflammation. Inna Viktorivna Buvailo is a very good specialist and a pleasant person, identified the cause of the postoperative pain and is helping with its treatment; the block was performed together with V.V. Beloshitsky.
They also gave an injection into the wrist (de Quervain syndrome), the result is good, the pain is gone.
Before that, I had seen many specialists in different clinics and I know how important it is when they are ready to listen to you and look for a solution.

Electroneuromyography (ENMG): what the test is, what happens and what it shows

Numbness, tingling, burning or weakness in the arms or legs, or pain radiating from the neck or lower back into a limb may lead a doctor to prescribe ENMG. Below, we explain what the test is, what it can show and how to prepare.

What is ENMG?

Electroneuromyography examines peripheral nerves and muscles. It tests the nerves of the arms, legs and face, nerve roots, transmission of signals from nerves to muscles, and the muscles themselves. ENMG has two parts.

  • Nerve conduction studies (nerve conduction studies). Adhesive electrodes are placed on the skin, a brief, weak impulse is delivered to a nerve, and the equipment records the response. This assesses how quickly and effectively the nerve conducts a signal.
  • Needle EMG. A thin needle electrode is inserted into a muscle to record its electrical activity at rest and during contraction. This helps distinguish nerve damage from damage to the muscle itself.

At SPRAVNO, both parts are available, including within a single examination. The doctor decides which nerves and muscles to test and whether needle EMG is needed.

APPOINTMENTS AND QUESTIONS
Enquire about an appointment
Leave your number for the administrator to contact you about a referred test or a doctor’s consultation. A consultation is a separate service.

When is ENMG prescribed?

SPRAVNO doctors recommend ENMG when there are signs of peripheral nerve or muscle dysfunction:

  • numbness, tingling, burning or reduced sensation in the arms or legs;
  • muscle weakness, loss of muscle bulk, difficulty holding objects or walking;
  • neck or lower back pain radiating into a limb, accompanied by numbness or weakness;
  • changes in sensation or movement after a nerve injury.

Studies in ENMG laboratories in Mexico, Turkey, Korea, Italy and Ethiopia report the same most common reasons for referral: carpal tunnel syndrome (12–62% of referrals), polyneuropathy (up to 34%) and radiculopathy — nerve root damage (10–35%).

A symptom alone is not a diagnosis and does not mean ENMG is needed. The test is always prescribed by a doctor.

What ENMG does and does not show

ENMG helps clarify the exact location, nature and severity of a problem. The results can help the doctor distinguish damage to the nerve’s covering from damage to the nerve fibres themselves and assess the prognosis.

Limitations of the test:

  • ENMG does not assess the brain or spinal cord. It therefore does not diagnose conditions such as multiple sclerosis, which is diagnosed using MRI and other methods.
  • Standard ENMG does not detect small-fibre nerve damage: results may be normal in this type of neuropathy.
  • Nerve conduction studies alone are insufficient to assess a nerve root: needle EMG is needed, and even that does not detect radiculopathy in every case.

ENMG and MRI do not replace each other. MRI shows structure — the spine, a disc herniation and tissues — while ENMG shows nerve and muscle function. MRI often detects changes that cause no symptoms, and it does not assess polyneuropathy.

Studies in ENMG laboratories report normal results in 25–38% of patients. The medical conclusion is always assessed alongside symptoms and examination findings.

What happens during the test

  1. The doctor asks about your symptoms, reviews the referral and previous test results, and determines the scope.
  2. Nerve conduction studies: electrodes on the skin and brief impulses. These feel like tingling or muscle twitching — uncomfortable, but usually not painful.
  3. Needle EMG, if needed: a single-use needle electrode is inserted into several muscles. Insertion causes discomfort like an injection, which passes shortly after the needle is removed. Medical intervention is performed only with the patient’s consent, as required by Article 43 of the Fundamentals of Ukrainian Legislation on Health Care.
  4. The doctor explains the results during the test, and you receive the medical report immediately.

The test can take up to 2 hours, depending on its type and scope. If anything is uncomfortable, say so and the test will be stopped or paused.

After the test, you can return to your usual activities immediately, including driving. After needle EMG, the muscles may feel tender for a few days and a small bruise may develop. If pain at the needle site increases, or swelling or pus develops, contact a doctor.

What an ENMG report looks like

The report includes tables for each nerve and muscle, limb temperature, reference values and a medical conclusion; tracings are included when needed. The diagrams below explain the main measurements.

latency, ms amplitude, mV impulse
The muscle’s response to an impulse. Latency is the time from impulse to response; a prolonged latency indicates damage to the nerve’s covering. Amplitude reflects the number of functioning nerve fibres.
M-response F-waves ≈ 20 ms
F-waves. Small late waves appear after the main response: the impulse has travelled to the spinal cord and returned. This assesses the parts of the nerve closer to the spine.
Rest Contraction
Needle EMG. A healthy muscle is electrically “silent” at rest. During contraction, motor unit potentials appear; the doctor assesses their shape, size and recruitment.

Illustrative diagrams, not an actual test.

How to prepare

  • On the day of the test, do not apply creams, oils or lotions to the areas being examined.
  • Wear loose clothing to allow easy access to your arms and legs.
  • Keep your limbs warm: wear gloves and warm socks in cold weather.
  • Eat and drink as usual; you do not need to fast.
  • Do not stop medicines on your own. If the test is to diagnose myasthenia, ask your doctor in advance how to take your medicines.
  • Bring your referral, previous ENMG reports, neurologist reports, other test results and a list of medicines.

Why warmth matters: cooling a limb slows nerve signal conduction — velocity falls by 1.5–2.5 m/s for each degree of cooling, which can make the result appear worse than it is.

Safety and precautions

  • Tell the doctor in advance about a pacemaker, implanted defibrillator or neurostimulator. Nerve conduction studies are usually safe with modern pacemakers.
  • Medicines that reduce blood clotting are not routinely stopped, but you must tell the doctor about them and any bleeding disorder, especially before needle EMG. Significant bleeding occurs in fewer than 1 in 10 000 tests.
  • The needle is not inserted into an area with an active infection. Report any wounds, rash or skin inflammation in the test area before the examination begins.
  • Pregnancy alone is not a contraindication to the test.
  • Needle electrodes are single-use.

ENMG after a nerve injury or wound

In the first week after an injury, ENMG may show the location of the damage and whether it is complete.

Signs of nerve fibre damage appear on ENMG 2–4 weeks after injury; earlier testing is performed only at the doctor’s discretion.

After 3–4 months, repeat ENMG shows whether there are signs of recovery. If there are none, doctors suspect complete nerve rupture and discuss surgery.

Motor recovery usually reaches a plateau after 18–24 months, so patients with severe injuries are referred to a surgeon early.

At the Romodanov Institute of Neurosurgery, researchers examined 480 patients with gunshot and mine-blast injuries to limb nerves, performing 1400 ENMG and EMG tests (Ukrainian Neurosurgical Journal, 2024). One of the co-authors is SPRAVNO doctor Oksana Mytsak.

Repeat ENMG

Repeat testing is performed to assess changes over time: after an injury, with new symptoms, when the diagnosis is unclear or the first test was incomplete.

Bring previous reports: the medical conclusion describes changes compared with the previous test.

In trials, different doctors measuring the same nerves obtained noticeably different values; standardising the technique reduces variation. For comparison, it is therefore important that the same testing method is used.
